A facility operating under Centers Health Care was investigated by the New York Attorney General in 2024 and the operator agreed to a $45 million settlement intended to address allegations involving resident neglect and chronic understaffing. Based on our family’s experience, many of those same concerns appear to remain ongoing today. The NY State Department of Health completed its annual survey of the facility in January 2026, yet returned only weeks later and found the facility out of regulatory compliance requiring corrective action. We also experienced ongoing unprofessionalism and obstruction involving Medical Records, Administration, and “Special Projects” personnel, including repeated refusal to provide a complete designated medical record set despite payment, documentation, and prolonged follow-up efforts. I strongly urge anyone considering any facility operated under this chain to thoroughly research the 2024 Attorney General lawsuit, advocate aggressively for your loved ones, request everything in writing, do not wait to obtain medical records, and involve your local ombudsman office and elected officials early. Families deserve transparency, accountability, and dignity for their loved ones.
