1. Agreement to these Terms
These Terms of Use (“Terms”) govern your access to and use of the CareIndex website and related services (the “Service”).
By accessing or using the Service, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Service.
2. What CareIndex provides
CareIndex is an online healthcare directory and discovery tool. We help people browse publicly available and user-submitted information about doctors, clinics, specialties, procedures, locations, and related signals. We do not practice medicine, nursing, pharmacy, or any licensed healthcare profession.
The Service helps users discover and compare publicly displayed information about healthcare providers and clinics. Primary actions on the directory are informational—for example, viewing profiles, reading reviews, browsing specialties, and finding locations.
Unless we clearly state otherwise in writing for a specific feature, CareIndex does not schedule appointments, deliver telehealth, process insurance claims, or act as a broker for medical services.
Directory information can change without notice. Before visiting any provider, confirm insurance participation, fees, hours, credentials, languages, availability, and services directly with that provider or their office.
3. No medical advice or provider relationship
Nothing on this website is medical advice, a diagnosis, a treatment plan, or a recommendation to use or avoid any provider, medication, test, or procedure. Always seek advice from a licensed clinician who knows your situation.
Using this website does not create a doctor–patient, clinician–patient, or any other healthcare provider relationship between you and CareIndex or between you and any listed provider.
If you think you may have a medical emergency, call 911 (or your local emergency number) immediately. Do not use this website for urgent or emergency care.
4. Eligibility and accounts
You must be at least 13 years old to use the Service. If you are under 18, you may use the Service only with permission of a parent or legal guardian.
You are responsible for maintaining the confidentiality of your account credentials and for activity under your account.
You agree to provide accurate account information and to update it when it changes.
5. Directory listings and provider content
Profiles may be created from public records, third-party sources, or information submitted by providers or authorized representatives.
We do not guarantee that any listing is complete, current, licensed, in-network, accepting patients, or appropriate for your needs.
Providers—not CareIndex—are responsible for compliance with licensing, advertising, insurance, and professional rules that apply to their practice.
We may edit, refuse, reorder, or remove listings that violate these Terms, appear fraudulent, or create legal or safety risk.
6. Reviews, ratings, and user content
If you submit reviews, ratings, or other content, you represent that your statements are based on your honest experience or lawful source, and that you will not post content that is false, defamatory, harassing, discriminatory, or invasive of others’ privacy.
Ratings, reviews, scores, rankings, wait-time estimates, and similar signals are informational only. They are not clinical quality measures, accreditations, endorsements, or guarantees of outcomes, safety, or suitability for your care.
We may moderate, remove, or decline to publish user content at our discretion. We are not obligated to host any particular review.
You grant CareIndex a non-exclusive, worldwide, royalty-free license to use, display, reproduce, and adapt your submitted content in connection with operating and promoting the Service.
7. Acceptable use
You agree not to:
- Use the Service for unlawful, harmful, or fraudulent purposes.
- Scrape, crawl, or harvest data in a way that burdens our systems or violates robots rules, except as allowed by law.
- Impersonate any person or misrepresent your affiliation with a provider or CareIndex.
- Attempt to gain unauthorized access to accounts, listings, or backend systems.
- Post protected health information about other people without lawful authority.
- Use the Service to market unapproved treatments, illegal drugs, or deceptive healthcare claims.
- Reverse engineer or interfere with security or integrity of the Service.
8. Intellectual property
The Service, including its design, logos, compilation of listings, and original content, is owned by CareIndex or its licensors and protected by intellectual property laws.
You may view and print reasonable portions of the Service for personal, non-commercial use. You may not copy, resell, or republish the directory as a competing product without written permission.
9. Third-party links and services
The Service may link to third-party websites, maps, phone numbers, or tools. We do not control and are not responsible for third-party content, privacy practices, or transactions you enter into off-site.
Your use of third-party services is at your own risk and subject to their terms.
10. Insurance and pricing information
Insurance labels shown in the directory are informational and may be incomplete or outdated. They are not a guarantee of coverage, network status, or payment. Only your insurer and the provider can confirm benefits.
11. Disclaimers
THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE MAXIMUM EXTENT PERMITTED BY LAW, CAREINDEX DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
We do not warrant that listings, scores, maps, wait-time signals, or search results will be accurate, uninterrupted, or error-free.

12. Limitation of liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, CAREINDEX AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND SUPPLIERS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, DATA, GOODWILL, OR HEALTH OUTCOMES, ARISING FROM YOUR USE OF THE SERVICE.
OUR TOTAL LIABILITY FOR ANY CLAIM RELATING TO THE SERVICE WILL NOT EXCEED THE GREATER OF (A) USD $100 OR (B) THE AMOUNT YOU PAID US, IF ANY, IN THE TWELVE MONTHS BEFORE THE CLAIM.
Some jurisdictions do not allow certain limitations, so some of the above may not apply to you.
13. Indemnification
You agree to defend, indemnify, and hold harmless CareIndex and its affiliates from claims, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your use of the Service, your content, or your violation of these Terms or applicable law.
14. Suspension and termination
We may suspend or terminate access to the Service at any time for violation of these Terms, risk to users, or operational reasons.
Sections that by nature should survive termination—including disclaimers, limitations of liability, and indemnity—will survive.
15. Governing law and disputes
These Terms are governed by the laws of the State of New York, United States, without regard to conflict-of-law principles, except where mandatory consumer protection laws in your jurisdiction apply.
Except where prohibited, you agree that courts located in New York County, New York will have exclusive jurisdiction over disputes arising from these Terms or the Service, unless we agree otherwise in writing.
