$15 to complete the Universal Child Healthcare form and $30 if you need them to do it same day. I've never in my life been charged for a standard form that they should be completing when you go to your child's annual check up. The Dr's are fine but they make all the day to day paperwork really difficult. Would not recommend.

They’ve always responded with a call after hours and got us in for sick visits quickly. The nurses and doctors are great with the kids and they answer all of our questions and we never feel rushed.
