I’ve been a patient of Dr. Yanina Kotlyar for about 5 years and I have always had a wonderful experience. She is extremely professional, caring, and attentive to every detail. You can really tell that she cares about her patients and takes pride in her work. Her staff is also fantastic—very friendly, organized, and welcoming every time I visit. The whole team makes the experience smooth and comfortable. I highly recommend Dr. Yanina Kotlyar to anyone looking for a trustworthy doctor with a great team. PLEASE AVOID DR. KOTLYAR and this practice at all costs. Long story short: 1. She instructs the front staff to tell patients they can have appointments after 5pm with her, which she never intends to keep. She leaves for the day when she feels like it and instructs the staff to book patients with a different provider without your consent. You will be in for a rude surprise when an unknown male walks into your exam room without introducing himself and he will also access your medical records without your consent (7/21/25). 2. She falsely billed for services that weren't done. 3. She billed for diagnoses that I never complained about. 1. The office will book you with another provider without your consent, all while lying to you that you are booked with Dr. Kotlyar. They will tell you that you have an appointment with her after 5pm but the doctor leaves for the day when she feels like it. She has done this twice, 1st on 7/21/25, and even after she told her staff that would stay past 5pm for my rescheduled appointment on 7/24/25, she cancelled that too. No, the staff will not call to let you know that the doctor has already left the building and didn't intend to stay for the appointment. Yes, the staff will gaslight you about changing your provider without consent - they will tell you "You never told us we couldn't change your provider." Even after they told me they put a note in my chart stating that I only want to see Dr. Kotlyar. They will tell you they put a chart note, but instead they put a note in the appointment calendar, not your chart. So, the office manager said the only way I can keep an appointment with Dr. Kotlyar, without them changing my provider, is telling them the front staff each time I book an appointment "Please don't change my provider." I have worked for several doctor's offices and have never switched my patient's providers without my consent. We always called to let the patient know that doctor's needed to reschedule or leave. We never lied to them about who they were seeing. 2. They are billing for procedures that were never done - I came in to get my blood drawn on 6/12/25 at the office. They billed for the blood draw but they falsely billed for a follow up visit cpt 99213 and urine dipstick cpt 81002, both which were NEVER done. I tried calling the office about getting this removed and they refused to help. They referred you to their billing company which you can never reach anyone. They didn't even call back my insurance company reps when they left several voicemails. As of 10/10/25 they have not taken off the 99213 and 81002. 3. They are using diagnosis codes that I never mentioned - they billed for palpitations and wheezing which was something I NEVER MENTIONED OR COMPLAINED ABOUT. I do not have heart or breathing problems. They're using this because they are ordering unnecessary testing - they take you to do an EKG and breath test even before you see the doctor for a new patient visit. Doctor's should be ordering testing AFTER they speak to patients, not before. I went to a new PCP and she was shocked I had an EKG 4. I had bloodwork on 6/12/25. After Dr. Kotlyar failed to keep my follow ups on 7/21/25 and 7/24/25, I asked on 7/24/25, 7/9/2 and 8/4/25 for a call back with my bloodwork results. The nurse finally called back on 8/6/25 with the results and had the audacity to tell me "we won't charge you for this phone call." The doctor failed to keep both of my follow ups and I waited 8 weeks to get my blood work results. 5. When I called the office later 8/4/25, I asked secretary Kate the name of the male doctor and she informed me it was Dr. William Schmalz. According to the CMS website, his specialty is general surgery. I do not know why he is working as a primary care physician. 6. The doctor was 1.5 hours late to my new patient appointment. The office messed up scheduling me - I asked them for the 2:15 pm appointment and they put me in at 11 am. No, they wouldn't honor the 2:15 pm appointment. Since the doctor was so late, I had to take a full day PTO instead of half.
