Was happy to work with a psychologist at wellness road psychology. They contacted me and matched me with a provider and provided me with a appointment for a consultation. Wellness Road Psychology sent a link to fill out information related to medical and behavioral history. They contacted me the following day and told me that the information I provided caused the Wellness Road Psychology to cancel the intake consultation with the psychologist. In the email they provided they did not even indicate whether or not the psychologist even reviewed my intake information. It just sounds like the intake coordinator reviewed the information and canceled the intake consultation without any feedback or judgement from the psychologist. Better to just lie on the the intake forms.
