This complaint is against Cityblock Health in Southfield, Michigan. I have been calling Cityblock six times since March about them sending my four consent forms to my house. I signed the consent forms during an online video call from my appointment with Dr. Tracey Alphin on March 17th. The appointment was concerning medications for my asthma. City block has not mailed them since. Some of the employees at Cityblock have claimed that the forms were already sent off but they are not in my mailbox. Also, the employees at Cityblock's said that they would have someone call me back concerning the forms but no one has called me back. I would like your company to force Cityblock health to mail off my forms to my house.
